Title: Extensive Methylation Analysis of Circulating Tumor DNA in the Plasma of Patients with Gastric Cancer
Objective
Specific and highly sensitive biomarkers for gastric cancer have yet to be established. In recent years, tumor-derived DNA present in peripheral blood, known as circulating tumor DNA (ctDNA), has attracted considerable attention across various cancer types. We have previously reported the utility of mutation-based ctDNA detection for assessing disease status in patients with gastric cancer. However, because mutations in specific cancer-associated genes, including TP53, are detected at relatively low frequencies in gastric cancer, the sensitivity of mutation-based ctDNA analysis is limited. We therefore focused on ctDNA methylation, hypothesizing that its detection could improve the sensitivity of liquid biopsy. In this study, next-generation sequencing (NGS) covering a broad range of CpG sites was performed using cancerous and noncancerous tissues from resected gastric cancer specimens, as well as plasma samples from patients with gastric cancer. We investigated whether ctDNA methylation levels could serve as a sensitive blood-based biomarker that accurately reflects disease status.
Methods and Results
Targeted bisulfite sequencing using QIAseq Targeted DNA Panels (QIAGEN) was employed for methylation analysis. Extracted DNA was subjected to bisulfite conversion using the EpiTect Fast Bisulfite Sequencing Conversion Kit (QIAGEN), followed by methylation analysis and sequencing on the MiSeq System (Illumina). The DNA methylation rate at each CpG site was calculated as a β-value by dividing the number of reads showing methylation at that site by the total number of reads matching the corresponding sequence context. The β-value for each gene was defined as the mean β-value of all CpG sites included in that gene.
The analysis initially included 63 genes previously reported to be methylated in malignant tumors, particularly colorectal cancer. Their methylation levels in gastric cancer were first evaluated using data from The Cancer Genome Atlas (TCGA). β-values for these 63 genes, measured in gastric cancer and noncancerous tissues using the Illumina HumanMethylation27 or HumanMethylation450 platform, were extracted from the TCGA database and analyzed. Of the 63 genes, 44 exhibited significantly higher β-values in cancerous tissues than in noncancerous tissues, confirming their hypermethylation. These 44 genes were therefore selected for subsequent analysis.
Next, DNA was extracted from cancerous and noncancerous tissues in resected specimens from 10 patients who experienced postoperative recurrence of gastric cancer using the GeneRead DNA FFPE Kit (QIAGEN). Methylation analysis of the 44 genes was then performed. Clustering analysis based on the ratio of β-values in cancerous versus noncancerous tissues (T/N ratio) generated four major clusters: A, B, C, and D. Among these, the five genes comprising cluster A—SPG20, FBN1, SDC2, TFPI2, and SEPT9—had the highest mean T/N ratio (A: 3.424; B: 1.693; C: 1.827; D: 1.120). These five genes were therefore identified as candidate targets for liquid biopsy using peripheral blood from patients with gastric cancer.
Subsequently, 16 patients who experienced postoperative recurrence and disease progression were analyzed. DNA was extracted from plasma samples collected at the time of disease progression using the Apostle MiniMax High Efficiency cfDNA Isolation Kit (Beckman Coulter), followed by NGS analysis. Clustering analysis based on the methylation levels of the five selected genes divided the patients into a high-methylation cluster (mean β-value: 0.2159) and a low-methylation cluster (mean β-value: 0.0418). Overall survival was significantly poorer in the high-methylation group than in the low-methylation group (log-rank P = 0.009). In contrast, when overall survival was compared between groups with high and low serum CEA or CA19-9 levels measured during the same period, no significant difference was observed (log-rank P = 0.208).
Finally, serial plasma samples collected at multiple time points were analyzed in one patient with clinical stage III gastric cancer who experienced recurrence after surgical resection. Although CEA and CA19-9 remained within their respective reference ranges throughout the clinical course, the mean methylation level of the five genes was highest before treatment, decreased following preoperative chemotherapy and surgery, and increased again at the time of recurrence. These findings suggest that ctDNA methylation levels of these five genes may reflect changes in tumor burden and disease status.
Conclusion
In this study, plasma ctDNA methylation levels were measured in patients with gastric cancer. The plasma methylation levels of five genes—SPG20, FBN1, SDC2, TFPI2, and SEPT9—were found to reflect tumor status and prognosis in patients with recurrent gastric cancer. Plasma ctDNA methylation analysis of these five genes may therefore represent a useful novel liquid-biopsy approach for gastric cancer.

〔目 的(Objective)〕
胃癌において特異的かつ高感度なバイオマーカーは未だ確立されていない。近年、末梢血中に存在する腫瘍由来
DNA(circulating tumor DNA: ctDNA)が各癌腫で注目されており、我々もこれまでに変異解析を用いた胃癌患者
の病勢評価におけるctDNA検出の有用性を報告してきた。しかし、胃癌ではTP53をはじめとする特定の癌関連遺伝
子の変異検出率が低いため、遺伝子変異に基づくctDNA解析は感度に限界がある。そこで我々は、ctDNAのメチル化
に着目し、その検出がリキッドバイオプシーにおける感度向上につながると考えた。本研究では、胃癌切除標本の
癌部・非癌部組織および胃癌患者の血漿を用いて、広範なCpG部位を網羅する次世代シークエンサー(NGS)解析を
行い、ctDNAのメチル化レベルが病勢を的確に反映し得る高感度な血中バイオマーカーとなるかどうか検討した。
〔方法ならびに成績(Methods/Results)〕
本研究のメチル化解析には、QIAseq Targeted DNA Panels(QIAGEN)を用いたターゲットバイサルファイトシー
クエンスを採用した。抽出したDNAをEPITECT Fast Bisulfite Sequencing Conversion Kit(QIAGEN)を用いてバ
イサルファイト処理し、メチル化解析を行った。その後、MiSeq System(Illumina)によりシークエンスを実施し
た。各CpGサイトにおけるDNAメチル化率は、当該部位でメチル化が認められたリード数を、そのコンテキストに一
致するリード数で割ったβ値として算出し、各遺伝子のβ値は含まれるCpGサイトの平均値とした。
解析対象としたのは、大腸癌を中心とした悪性腫瘍におけるメチル化が報告されている63遺伝子であり、まず
The Cancer Genome Atlas(TCGA)データベースを用いて胃癌におけるメチル化レベルを測定した。TCGAデータベー
スより,Illumina Human Methylation 27/450 プラットフォームにより測定された 63遺伝子の胃癌部および非癌部
のβ値を抽出し、解析を行った。その結果、44遺伝子が非癌部と比較して癌部で有意に高いβ値を示し、高メチル
化されていることが確認されたため、以後はこれら44遺伝子を解析することにした。
次に、胃癌術後再発患者10例を対象に、GeneRead DNA FFPE Kit (QIAGEN)を用いて、切除標本から癌部および非
癌部のDNAを抽出し、44遺伝子のメチル化解析を行った。癌部/非癌部のβ値の比(T/N)を用いたクラスタリング解
析の結果、4つの主要クラスター(A、B、C、D)が生成され、その中でもクラスターAを構成しているSPG20, FBN1,
SDC2, TFPI2, SEPT9の5遺伝子はT/Nの平均値が最も高値であったため(A:3.424、B:1.693、C:1.827、D:
1.120)、これら5遺伝子を胃癌患者末梢血を用いたリキッドバイオプシーの候補遺伝子と考えた。
そこで、術後再発・増悪の経過をたどった16例を対象に、病勢増悪時に採取した血漿からApostle MiniMax High
Efficiency cfDNA Isolation Kit (Beckman Coulter)を使用してDNA抽出を行い、NGS解析を行った。上記5遺伝子
のメチル化レベルをもとにクラスタリング解析を行うと、高メチル化クラスター(平均β値:0.2159)と低メチル化
クラスター(平均β値:0.0418)に二分され、高メチル化症例では低メチル化症例に比べて全生存期間が有意に不良
であった(log-rank P=0.009)。一方で、同時期に測定した血中のCEAまたはCA19-9の高値群と低値群の全生存期
間を比較した場合には、両群間に有意差は認めなかった(log-rank P=0.208)。
最後に、cStage III胃癌に対して切除後に再発した1症例において複数時点での血漿を採取して解析したとこ
ろ、CEAやCA19-9が経過を通じて基準値内に留まったのに対し、5遺伝子の平均メチル化レベルは治療前に最も高値
を示し、術前化学療法および手術により減少、その後再発時に再上昇する経過を示した。この結果から、これら5
遺伝子のctDNAメチル化レベルが腫瘍の病勢を反映している可能性が示唆された。
〔総 括(Conclusion)〕
本研究では、胃癌患者を対象に血漿中ctDNAのメチル化レベルを測定したところ、5つの遺伝子(SPG20, FBN1,
SDC2, TFPI2, SEPT9)の血漿中メチル化レベルが、再発胃癌患者における腫瘍病勢や予後を反映することを明らか
にした。これら5遺伝子の血漿ctDNAメチル化解析は、胃癌に対する新たなリキッドバイオプシーとして有用である
可能性が示唆された。
